If your Florida license plate or registration decal has been lost or stolen, you may request a replacement by completing the required application and providing proof of identity.
To request a replacement, submit the Application for Replacement License Plate, Validation Decal, or Parking Permit (Form HSMV 83146)together with a copy of the registered owner's valid driver license.
Replacement fees apply when a license plate or decal has been lost.
If the license plate or decal was stolen, no replacement fee will be charged provided you submit a law enforcement report or agency-issued card that includes the case number.
