Any person with a blue permanent disabled parking permit must provide a certificate of disability completed and signed by a certifying authoritywithin the last 12 months in order to renew the permit. To renew your permanent disabled parking permit, complete Form HSMV 83039and mail it with your renewal notice to the Miami-Dade County Office of the Tax Collector.
If your disabled parking permit has been lost or stolen, submit Form HSMV 83039together with Form HSMV 83146to request a replacement.
Permanent disabled parking permits may only be renewed within three (3) months before the expiration date. The certifying authority's signature must be dated within the previous 12 months.
If your permit is lost or stolen within 12 months of the original issue date, you only need to submit Form HSMV 83146along with a valid copy of your Florida driver license or identification card.
If more than 12 months have passed since the permit was issued, you must submit a new Form HSMV 83039signed by a certifying authority, together with Form HSMV 83146and a valid copy of your Florida driver license or identification card.
